Accepted status documents include your Permanent Resident Card (Form I-551, the green card), an Employment Authorization Document (Form I-766), or a Form I-94. All documents must be originals or copies certified by the issuing agency, not photocopies.

    Apply for a corrected Social Security card in your new name with Form SS-5. There's no fee. Your marriage document supports a surname change (for example, taking your spouse's last name or hyphenating the two). Bring or mail your originals to your local Social Security office.
