Name Change After Marriage for Seniors and Retirees
If you are a senior or retiree, the government process for changing your name after marriage is the same as for anyone: update your Social Security record first using Form SS-5 (no fee), then your driver's license and REAL ID, then your passport. The main additional considerations are Medicare and retirement accounts. Medicare instructs beneficiaries to contact Social Security for a name change — you do not file a separate Medicare form. Your retirement payment amounts are not affected by a name change. Pensions, IRAs, and other financial accounts each need to be notified separately.

Change your name with Social Security (free) — do this first
Apply for a corrected Social Security card with Form SS-5. There is no fee. Your name change at Social Security also updates your Social Security retirement record. Bring your marriage certificate and proof of identity to your local SSA office, or mail originals according to SSA's instructions. Documents must be originals or agency-certified copies, not photocopies.
