- Is DS-5504 really free?
- Yes. Within 1 year of your passport's issuance, the State Department charges no fee for a name-change correction with DS-5504. You only pay $60 if you want it expedited.
- What do I send with the form?
- Your most recent passport, an original or certified marriage certificate, and (for DS-5504) one passport photo. Photocopies of the marriage certificate aren't accepted.
- Same forms after a divorce?
- Yes — the DS-5504, DS-82, and DS-11 form logic is identical. The one thing that changes is the document that traces your old name to your new one: a certified divorce decree instead of a marriage certificate.
